ACM Healthcare is on the lookout for a compassionate and dedicated Registered Nurse to join our client’s Residential Rehabilitation team in Helensvale. The Registered Nurse will play a vital role in providing high-quality care to clients throughout their rehabilitation journey. Reporting to the Medical Experience Manager you will conduct comprehensive assessments, deliver skilled nursing

interventions, educate client’s and families, and collaborate with the interdisciplinary team to ensure coordinated care.

 

RESPONSIBILITIES:

  • Client Assessments: Perform thorough assessments to evaluate a client's risk profile, medical history, current functional abilities, social support systems, and potential discharge needs.
  • Care Plan Development: Work collaboratively with the interdisciplinary team (including physicians, therapists, nurses, and social workers) to create personalised care plans that address the physical, emotional, and social needs of the client.
  • Plan Monitoring and Adjustment: Continuously monitor clients' progress toward their goals, identify any challenges, and adjust the care plan as necessary in collaboration with the team.
  • Facilitate Communication: Ensure effective communication between clients, families, and all healthcare team members, keeping everyone aligned with the care plan.
  • Coordinate Referrals: Facilitate timely referrals to appropriate specialists, community resources, and support services that can aid in the clients recovery.
  • Discharge Planning: Actively engage in discharge planning by identifying suitable post-rehabilitation settings and services, such as home care or assisted living, based on the client's needs.
  • Empowering Clients: Educate clients and their families about the rehabilitation program, treatment options, potential challenges, and expected outcomes, empowering them to actively participate in decision-making.
  • Reporting: Generate reports for physicians, therapists, and other healthcare professionals to keep them informed about the client’s progress.

 

MANDATORY QUALIFICATIONS AND EXPERIENCE:

  • Registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
  • Extensive experience working in a healthcare setting, preferably in a residential rehabilitation environment.
  • A valid Australian Drivers License
  • Strong understanding of adult medical, nursing principles and rehabilitation practices.
  • Compassionate and client-centered, trauma informed approach to care.
  • Ability to work independently and as part of a team.
