ACM Healthcare is on the lookout for a compassionate and dedicated Case Experience Lead to join our client’s Residential Rehabilitation team in Helensvale. Play a vital role in supporting clients throughout their rehabilitation journey. The Case Experience Lead will conduct comprehensive assessments, develop individualised care plans, and coordinate the continuation of care across the interdisciplinary team. Additionally, they will empower clients and families by providing education, support, and advocating for their needs.
RESPONSIBILITIES:
- Client Assessments: Contribute to comprehensive assessments to evaluate a client's medical history, current functional abilities, social support systems, and potential discharge needs
- Care Plan Development: Collaborate with the interdisciplinary team (physicians, therapists, nurses, social workers) to create individualised care plans that address the client's physical, emotional, and social needs.
- Plan Monitoring and Adjustment: Regularly track the client’s progress towards their goals, identify challenges, and adjust the care plan as necessary in coordination with the team
- Communication Facilitation: Ensure safe and consensual communication between clients, families, and all members of the healthcare team, keeping everyone informed and aligned.
- Referrals Coordination: Coordinate and facilitate timely referrals to appropriate specialists, community resources, and support services that can aid in the client’s recovery.
- Discharge Planning: Actively engage in discharge planning, identifying suitable post-rehabilitation settings and services (e.g., home care, assisted living) based on the client’s needs.
- Client Empowerment: Educate clients and families about the rehabilitation program, treatment options, potential challenges, and expected outcomes. Empower them to actively participate in decision-making and understand their role in the recovery process.
- Team Leadership: Lead a team of carers by setting clear expectations, evaluating performance, providing constructive feedback, fostering a positive work environment through recognition, and resolving conflicts effectively to maintain team harmony.
- Reporting: Prepare reports for physicians, therapists, and other healthcare professionals involved in the client's care, keeping them updated on the client’s progress.
MANDATORY QUALIFICATIONS AND EXPERIENCE:
- Relevant tertiary qualifications in Social Work and eligibility for membership with AASW
- Mental Health Accreditation highly regarded
- A valid Australian Drivers License
- Strong understanding of the rehabilitation process and interdisciplinary care models.
