Role Overview: An opportunity is available for a Health Professional Level 2 (HP2) Social Worker to join our client’s Rehabilitation Aged and Community Services. The role includes working within and to support various rehabilitation services and collaborate with an interprofessional team.

Duties & Responsibilities:

• Support services including outpatient rehabilitation clinics, motor neurone disease services, and dementia rehabilitation services.
• Work collaboratively as part of the team to ensure comprehensive care for clients.
• Engage in professional development and clinical supervision opportunities.
• Provide field education and mentoring to Social Work students.
• Potential to rotate into other Social Work areas within RACS.

Criteria:

• Experience working in a healthcare environment is preferred but not essential.
• Qualification in Social Work with a minimum of 12 months professional experience.
