MENTAL HEALTH PROFESSIONALS - Clinical Care Coordinator – Townsville QLD – Government Department

  • $55-$65 per hour.
  • Full time, 37.5 hour a week role
  • Open to Psychologist, Mental health Social worker, Mental health Nurse or Mental health Occupational Therapist   

 
ACM Healthcare is working on behalf of the Federal Government Department to source a Clinical Care Coordinator to work within the veteran community. The role is based in Townsville QLD. It is a complex Case Manager position, experience working as a case worker usually needs to be a fully registered Psychologist, Mental health Accredited Social worker, Mental health Nurse or Mental health Occupational Therapist   


Job Description:

  • A strong emphasis on worker-client alliance, and the involvement of family and caregivers as indicated and permitted by the client. Supporting clients with complex case management and employs elements of focused psychological strategies which include psychoeducation, distress tolerance, problem-solving, and/or motivational interviewing to facilitate progress towards case management goals.
  • Liaise and form partnerships with government departments, ex-service organisations, community based mental health and welfare agencies, and all other organisations involved in meeting the identified needs of communities and families, in particular where there is a need for both acute care and longer term rehabilitation of clients. This includes the development of a shared knowledge base that helps inform appropriate practice and referral pathways for service access.
  • Participate in community development and service promotion activities as required.
  • Participate in program evaluation and research activities, contribute to project management activities that implement strategic direction and quality improvement recommendations, and provide feedback for ongoing development of clinical policies and procedures, particularly in relation to case management development activities.

  
Mandatory Requirements:

  • Qualifications must be that of a Psychologist, Occupational Therapist, Mental Health Social Worker, or Nurse with mental health experience.
  • Qualified Psychologists, Occupational Therapists and Nurses must have current registration with the Australian Health Practitioners Registration Agency (AHPRA).
  • Qualification as a Social Worker eligible for membership of the Australian Association Social Workers (AASW) and eligible to apply for Mental Health accreditation
  • Working with Children (WWC) and/or Vulnerable People Checks/Clearance held and maintained while employed
  • Drivers License
