ACM Healthcare is working on behalf of a Non for Profit and Registered Charity that specialises in partnering with local communities and empowering individuals to live their best lives. Creating a genuine impact for the community and beyond. The role of Team Leader is within the new Dementia Support Program, is permanent, full time 5 days a week with flexible working arrangements, workforce wellbeing support and salary sacrificing.  

 

About the Role:

The Dementia Specific Program strives to give people with mild to moderate dementia and their family carers psychosocial, emotional, and practical assistance, as well as day and overnight respite. The Team Leader will be responsible for aiding the Manager in the efficient running of the program and providing support and guidance to staff, volunteers, and students. This role will involve coordinating the daily operations of the program, including all centre activities, in collaboration with and under the direction of the Manager. 

  • Develop individual support plans for people with dementia and their carers
  • Manage service delivery, ensuring all support provided to clients is in line with program requirements
  • Organise group therapies in conjunction with the Therapist
  • Coordinate overnight respite based on the needs of the person living with dementia and their primary carer
  • Maintain an understanding of community/client needs and use this knowledge to plan and provide recommendations regarding the program

 

Criteria:

  • Minimum 3 years’ experience in a similar role
  • Tertiary education in Health, Social Work or Counselling
  • Experienced in delivering clinical interventions for people with mild to moderate dementia
  • Strong knowledge of the New Aged Care Quality Standards
  • Leadership experience through managing a small team
